মালেয়শিয়া থেকে একজন আত্নীয় কল দিলেন।
বললাম জ্বী বলুন।
-রোগী বললেন ওনার দুর্বল দুর্বল লাগে।বললাম এটা কী অল্প কাজ করার পর নাকি সবসময়ই এরকম মনে হয়।
– -অল্প কাজ করলেই দুর্বল লাগে।একমাস থেকেই এরকম হচ্ছে।
আর কোনো সমস্যা আছে?
-অন্যদের থেকে বেশি ঠান্ডা লাগে।এসির বাতাস সহ্য হয়না।
আর তেমন কোনো সমস্যা নেই।
আমি ওনাকে ,routine investigation CBC..ECG দিলাম।সাথে thyroid function test দিলাম
নিচের ফটোতে রেজাল্ট আছে।
TSH (Thyroid Stimulating Hormone): Elevated (6.07 mIU/L, above the normal range of 0.27–4.20 mIU/L)
free T3 and Free T4: Within normal range (Free T3: 5.97 pmol/L, Free T4: 18.3 )
TSH normal range থেকে বেশি।কিন্তু ft3..ft4 normal range এর মধ্যে।
এটাকে আমরা subclinical hypothyroidism বলি।
ওনার সামনে hypothyroidism develop করতে পারে।
যেহেতু ওনার দু একটা উপসর্গ আছে ,ওনার hypothyroid state এ যাবার রিস্ক বেশি।তবে এখনই ড্রাগ শুরু করার প্রয়োজন নেই।বইপত্র যা বলে।
ওনাকে একটা tpo antibody দিবো!
autoimmune thyroiditis exclude করতে।
আর আবার দু মাস পর TSH দেখবো।
দু মাস পর TSH যদি 10 এর বেশি আসে,তাহলে levothyroxin দিয়ে চিকিৎসা শুরু করা যাবে।
