Hypertensive পেশেন্টদের Amitriptyline না দেয়াই better. কারণ এটা HTN কে aggravate করতে পারে।
Amitriptyline NE reuptake inhibit করে। ফলে synaptic cleft এ NE এর concentration বেড়ে যায়। আর এই NE vascular Alpha-1 receptors এর উপর কাজ করে vasoconstriction করে। ফলে TPR বাড়ে, ফলে DBP বাড়ে। আবার এই NE, heart এর Beta-1 এর উপর কাজ করে SBP বাড়াতে পারে।
এখানে theoretical explanation এর পাশাপাশি আমাদেরকে Evidence based study ও দেখতে হবে। Clinically Amitriptyline দিলেই যে সবার HTN aggravate করে এমন দেখা যায়নি। তাই আপবি risk-benefit ratio হিসেব করে clinical decision নিবেন। As a clinician এটা আপনার right, আলহামদুলিল্লাহ।
