Chronic_Pancreatitis এ CHO , Protein , Fat , Fat soluble Vitamin সব কিছুর Malabsorption হলেও, Iron Malabsorption হয় না।
কারন Iron Absorption হতে দরকার হয় Intact_intestinal_lining , Pancreatic Enzyme এর সেখানে দরকার হয় না।
Inflammatory_Bowel_Diesease এ যেহেতু intestinal lining নষ্ট হয়ে যায়, তাই সেখানে কিন্তু iron malabsorption হয়ে থাকে। যার জন্য হতে পারে Iron deficiency.
90% exocine function loss না হলে steatorrhea and wt loss হয় না।
কি investigation দ্বারা বুঝতে পারবো :
fecal elastase কমে যাবে
fecal fat content বেড়ে যাবে
100% exocine function lost হয়ে গেলেও আমরা কিন্তু শুধু 10 % enzyme supplement দেই।
এই 10% এ steatorrhea prevent করতে পারে।
আমারা pancreatine enzyme যেটার মধ্যে amylase,protease,lipase combination থাকে সেটা supplement দেই।

1+1+1// 2+2+2// 2+1+2+1+2 এভাবে দেই পেশেন্ট এর প্রয়োজন অনুযায়ী। অবশ্যই এটা খাবারের সাথে দিতে হবে।
অবশ্যই যতদিন zymet খাবে ততদিন ই ppi খাবে।
zymet pH <4 হলে damage হয়ে যায় যেহেতু এটা একটা enzyme.

Indications of Tablet :-Zymet Pro 325 mg (AS A Pancreatin Digestive Enzyme).
Zymet Pro is employed for the treatment of patients with
- Chronic pancreatitis
- Cystic fibrosis/Mucoviscidosis
- Following partial or complete pancreatectomy
- Total gastrectomy
- Steatorrhea
- Somatostatinoma
- Celiac disease and
- Other conditions associated with pancreatic insufficiency
Composition
Each enteric film-coated tablet contains 325 mg of Pancreatin BP that provides not less than the following enzymatic activity-
- Amylase 7800 units
- Protease 455 units
- Lipase 6500 units
Pharmacology of Tablet :-Zymet Pro 325 mg (AS A Pancreatin Digestive Enzyme).
Pancreatin is a preparation of Pancreatin, an extract from mammalian pancreas containing enzymes with Amylase, Protease and Lipase activity. It is designed to disintegrate in the alkaline medium of the duodenum where it releases the active enzyme components of Pancreatin (Amylase, Protease and Lipase). Amylase hydrolyzes dietary starch to yield maltose, maltoriose and a-dextrin. Protease cleaves peptide bonds in the center of proteins and polypeptides and forms amino acids and oligopeptides. Lipase hydrolyzes dietary triglycerides forming two molecules of fatty acid and a molecule of beta-monoglyceride. It converts not less than 25 times its weight of starch into soluble carbohydrates and not less than 25 times its weight of casein into proteoses within 5 mins, (equivalent to 150 times in 30 mins).
Dosage & Administration of Tablet :-Zymet Pro 325 mg (AS A Pancreatin Digestive Enzyme).
2-3 tablets to be swallowed whole with little water immediately after or during meal. Or as directed by the physician
Interaction of Tablet :-Zymet Pro 325 mg (AS A Pancreatin Digestive Enzyme).
Alkaline media will rupture the enteric coating. As such to prevent bursting out of the content in the stomach Zymet Pro should not be used concurrently with antacid.
Contraindications of Tablet :-Zymet Pro 325 mg (AS A Pancreatin Digestive Enzyme).
Pancreatin is contraindicated in patients with known hypersensitivity to the drug.
Side Effects of Tablet :-Zymet Pro 325 mg (AS A Pancreatin Digestive Enzyme).
The most frequent side effects are gastrointestinal including nausea, vomiting and abdominal discomfort. Buccal and perianal soreness may occur, particularly in infants. Hypersensitivity reactions manifested by sneezing, lacrimation or skin rashes have been reported.
Pregnancy & Lactation of Tablet :-Zymet Pro 325 mg (AS A Pancreatin Digestive Enzyme).
Pancreatin should be given to a pregnant woman only if clearly needed. It is not known whether this drug is excreted in human milk. Because many drugs are excreted in human milk, caution should be exercised when Pancreatin is administered to a nursing mother.
Precautions & Warnings
Patients known to be allergic to animal protein should be treated with caution.
Overdose Effects of Tablet :-Zymet Pro 325 mg (AS A Pancreatin Digestive Enzyme).
Excessive dosage may produce a laxative effect. Systemic toxicity does not occur. Very high dosage may cause hyperuricaemia and hyperuricosuria
