Serum Amylase and Lipase কি?
উত্তর- amylase এবং lipase হচ্ছে দুইটা ডাইজেস্টিভ এনজাইম, pancreas এর Aciner cell থেকে এই দুইটা Enzymes Secret হয়, এবং fat digestion এ help করে.
যখন pancreas এ কোনো inflammation হয়, তখন পেটের বাম পাশে left hypochondriac region এ প্রচন্ড ব্যাথা হয়, সাথে খাওয়ার রুচি চলে যায়, বমি হতে পারে, ডায়েরিয়া থাকতে পারে-
৬০% পেশেন্টের জ্বর আসতে পারে, মূলত idiopathic cause and Gall stone এর কারণে acute pancreatitis হতে পারে.
pancreatitis হলে WBC count বেড়ে যাবে,
pancreas এ কোনো inflammation হলে pancreatic enzymes বিশেষ করে Aciner CEll থেকে প্রচুর পরিমান pancreatic Enzymes Amylase and Lipase ক্ষরণ হয়, এবং তা ব্লাডে চলে আসে,
স্বাভাবিক healthy মানুষের ব্লাডে একটা নির্দিষ্ট পরিমান serum amylase and lipase থাকতে পারে-
কিন্ত pancreas এ কোনো প্রদাহ হলে এই পরিমাণ বেড়ে যায়-
Acute pnacreatitis এ Serum amylase and lipase নরমাল ভ্যালু থেকে ৩ গুণ বেড়ে যায়
Serum Amylase আবার ইউরিনের সাথে বের হয় যায়,
তবে Serum lipase body তে বেশি সময় থাকে,
Serum lipase থেকে Serum amylase এর sensitivity কম,তাই Serum lipase er diagnostic value বেশী,
serum amylase পেট ব্যাথার ৬ ঘন্টার মধ্যে Raise করতে পারে, ১২ ঘন্টায় মধ্যে peak করে, ৫ দিন পরে Normal level এ আসতে পারে.
আবার Serum lipase ব্যাথা শুরু হবার ৬-১২ ঘন্টার মধ্যে raised করে, ২৪ ঘন্টায় peak level এ যায়, ৮-১৪ দিন পর্যন্ত elevated থাকতে পারে
তাই পেশেন্ট পেট ব্যাথা নিয়ে আসলে আর আপনি তা pancreatitis চিন্তা করতে Serum amylase /lipase দিতে চাইলে ৬ ঘন্টা পরে দিলেই ভালো।
এতে করে রিপোর্ট সঠিক আসার সম্ভাবনা বেশি.
দুইটার মধ্যে যদি একটা দিতে চান, তাহলে Serum lipase দিবেন।।
