Pharmacology Case No 17. Ondansetron Vs  Domperidone

আসুন, একটা ইন্টারেস্টিং বিষয় নিয়ে আলোচনা করি—–

আমরা পার্সোনাল লাইফ কিংবা চেম্বারে এন্টি এমিটিক হিসাবে যে কয়টা ড্রাগের নাম জানি, তার মধ্যে

Domperidone and Ondansetron (Emistat) অন্যতম।

এখন কথা হচ্ছে, দুইটাই এন্টি এমিটিক, কখন কোন টা দিবেন?

আমরা আগে দুইটার মেকানিজম অফ একশন টা একটু জেনে নিই-

Vomiting Centre ও Chemo receptor trigger zone থাকে সেরেবেলাম এর একটু নিচে…..

Mechanism of Vomiting :

Chemoreceptor zone যদি কোনো ভাবে stimulate হয়৷ তাহলে Nausea Vomiting হয়।

Chemoreceptor Trigger zone কিভাবে stimulate হয়?

1..এখানে কিছু স্পেশাল রিসেপ্টর আছে, এই রিস্পেটর গুলি stimulate হলে Chemoreceptor zone stimulate হয়.

আমরা জানবো–

দুইটা Receotor এর নাম, যার ৯৫% Vomiting এর জন্য Responsible :

1… Serotonin 5-HT-3 receptor

2… Dopamine (D2 receptor)

Serotonin 5 HT 3 recaptor কোথায় থাকে–

Brain steam এর Chemoreceptor trigger zone এ থাকে.

এইটার কাজ কি?

এইটা Stimulate হলে Nausea /Vomiting হয়-

5 HT-3 কিভাবে stimulate হয়?

1… Rota viral infection হলে 5 HT-3 Stimulate হয়, তাই Rota viral diarrhoea তে বমি হয়.

2…Food poisoning হলে 5HT-3 receptor stimulate হয়,

3…Morphine /Pethidine জাতীয় ঔষধ দিলে

4… Bad smelling form environment can stimulate 5HT3

5… Many toxin and drugs can stimulate 5HT-3

6… Dengue viral infection হলে অতিরিক্ত পরিমানে 5HT3 stimulate হয়,

এইরকম আরো অনেক Triggering factor রয়েছে।

যে কোনো কারণেই 5HT3 receptor stimulate হলে Nasea and Vomiting হয়……………

Dopamine (D2 receptor) stimulate হলেও Nausea Vomiting হয়,

D-2 receptor কিভাবে stimulate হয়?

যেইসব কারণে 5HT3 receptor stimulate হয়, সেইসব কারণে D2 receptor ও stimulate হতে পারে,তাই Nausea Vomiting হতে পারে

Gastric acidity এর কারণে D2 receptor stimulate হয়, তাই Gastritis এ Nausea Vomiting হয়

D2 receptor সমপর্কে একটা স্পেশাল পয়েন্ট নোট করে নিন.

D-2 receptor যেমন Centrally Brain এ থাকে, তেমনি GIT mucosa তেও D2 Receptor পাওয়া যায়…..

GIT তে D2 receptor এর কাজঃ

১… Delayed Gastro intestinal Motility…

অর্থাৎ D2 receptor stimulate হলে Stomach থেকে food Empty হতে টাইম লাগবে, ক্ষুধা কম লাগবে,

Acidity বাড়বে, Nausea হতে পারে।

পুরো আলোচনাতে আমরা ২ টা বিষয় পেলাম–

Nausea and Vomiting এর Most common mechanism হচ্ছে-

1.. Serotonin 5 HT 3 receptor stimulate হওয়া

2.. Dopamine (D2 receptor) stimulate হওয়া।

তাহলে বমি বন্ধ করার জন্য কি করবেন—–

1… 5 HT3 কে stimulate হওয়া বন্ধ করে দিন-

2… Dopamine D2 receptor কে stimulate হওয়া বন্ধ করে দিন।

যে ড্রাগস দিয়ে serotonin 5HT3 কে stimulate হওয়া বন্ধ করে দেওয়া যায়, তাকে বলে Serotonin 5-HT-3 antagonist :: উদাহরণ Ondansetron বাজারে Emistat, zofra,. ইত্যাদি নামে পাওয়া যায়।

যে ড্রাগস দিয়ে Dopamine D-2 receptor stimulate হওয়া বন্ধ করে দেওয়া যায়, তাকে বলে Dopamine antagonist : (Domperidone নামে পাওয়া যায়)

আমরা একটু আগে জেনেছি যে-

D-2 receptor stimulate হলে Gastritic Motility delay হয়,

তাহলে D-2 stimulation বন্ধ করে দিলে কি হবে?

1… Gastric motility বাড়বে,

2… তাড়াতাড়ি Stomach empty হবে, খাবার নিছের দিকে চলে যাবে, তাই Nausea and Vomiting কমবে,

3.. Stomach early empty হলে তাড়াতাড়ি ক্ষুধা লাগবে, (Keep it in your mind that,. Just এই কারণেই যাদের খাবার রুচি কম, তাদের কে Domperidone দেওয়া হয়, এবং Domperidone Early stomach empty করে খাবার রুচি বাড়ায়)

4… যেহেতু D2 receptor stimulate

হলে Intestinal motility decrease হয়, তাই Domperidone দিলে intestinal motility increase হবে,

ডায়রিয়ার চান্স থাকবে,

তাই ডায়েরিয়া+ vomiting থাকলে Domperidone must be avoided.

ডায়েরিয়া থাকা কালীন Domperidone দিলে intestinal motility বেড়ে যাবে।

special note যারা গাইনিতে প্র‍্যাকটিস করেন——

Domperidone প্রোলাক্টিন হরমোনের পরিমান বাড়ায়,

আর প্রোলাক্টিন ব্রেস্ট মিল্ক সিক্রেশন বাড়ায়,,,, তাই যেইসব নবজাতক মায়ের দুধ পায়না, কিংবা মায়ের দুধ কম তৈরি হয়, তাদের কে Domperidone 10 mg

1+1+1 দিলে for 28 days.

breast milk secretion বাড়বে.

Immediately after delivery / or C/S করার পর Oral ডায়েট শুরু করলে অবশ্যই Domperidone suggest করবে।

এবার জেনে নিই –

কোন টার Anti emetic action বেশি-

5-HT3 receptor antagonist এর এন্টি এমিটিক একশন বেশি.

5 HT-3 antagonist এর মধ্যে আছে

Ondansetron and palanosetron

Palanosteron : Paloxi নামে পাওয়া যায়, I/V and oral dose.

Palanosetron এর কার্যকরিতা Emistat (Ondansetron থেকে অনেক বেশি)

Ondansetron দৈনিক ৩ বার দেওয়া লাগে,

Palanosetron দৈনিক একবার দিলেই হয়।

Domperidone and Ondansetron এর মধ্যে Ondansetron এর কার্যকরিতা বেশি,

তাই active vomiting e drug of choice Ondansetron /palanosetron.

কোন ধরণের Nausea তে Domperidone দিবে?

Gastritis.. GI upset, drug induced Nausea ইত্যাদিতে Domperidone দিবে।

Motion sickness এর Vomiting এর মেকানিজম কিছুটা ব্যতিক্রম,

সেখানে Dizziness, drowsiness, +Visual sensory disturbance থাকে।

তাই এইসব ক্ষেত্রে এন্টি এমিটিক হিসাবে Anti histamine জাতীয় এন্টি এমিটিক চয়েজ করা হয়।

2.Domperidone and ppi 2 ta aksate newa jabe vai?

Answer is:-jabe

Tablet 1.Vertina Plus (25mg+50mg) (AS A (Meclizine+Pyridoxine).AS A Anti-emetic drugs).

Indications

Prevention and treatment of nausea, vomiting, dizziness, motion sickness, radiation sickness and vertigo associated with diseases of the vestibular system (e.g. Meniere’s syndrome, labyrinthitis and other vestibular disturbances).

Pharmacology

Meclizine is a piperazine-derivative antihistamine that is used as an antiemetic. It has antiemetic, anticholinergic and antihistaminic properties. It reduces the sensitivity of the labyrinthine apparatus. The action may be mediated through nerve pathways to the vomiting center (VC) from the chemoreceptor trigger zone (CTZ), peripheral nerve pathways, the VC, or other CNS centers. Pyridoxine is vitamin B-6. It has been added to enhance the anti-emetic effects & as a dietary suppliment.

Dosage & Administration

Adult and Children 12 years of age & over:

  • Nausea and vomiting: 25-50 mg daily or as directed by a physician.
  • Motion sickness: Take an initial dose of 25-50 mg, 1 hour prior to travel. May repeat the dose every 24 hours for the duration of the journey.
  • Radiation sickness: 50 mg administered 2-12 hours prior to radiation treatment.
  • Vertigo: 25-100 mg daily in divided doses.
  • Prevention of nausea and vomiting associated with emergency contraceptive pill (ECP): 25-50 mg, 1 hour before first ECP dose; repeat if needed in 24 hours.

The safety and efficacy for use in children less than 12 years of age have not been established.

Interaction

The CNS depressant effects of Meclizine can be potentiated by concurrent use of Ethanol or other CNS depressant agents such as Benzodiazepines, Barbiturates, Tricyclic antidepressants, opiate agonists, skeletal muscle relaxants and antihistamines. Concurrent use of other anticholinergics can potentiate the anticholinergic effects of Meclizine. Meclizine can increase the absorption of digoxin by decreasing gastrointestinal motility.

Contraindications

Meclizine Hydrochloride and Pyridoxine Hydrochloride is contraindicated in patients who are hypersensitive to these ingredients.

Side Effects

Drowsiness, dry mouth and, on rare occasions, blurred vision have been reported.
