Urine_R_E analysis..
শায়েলা ২৭ বছরের Non pregnant women, সাধারণত health checkup এর জন্য আসছে, কোনো উপসর্গ নাই,
তার CBC report সব নরমাল,
Urine R/E = Pus cell 20 hpF
নরমালের চেয়ে ৪ গুন বেশী, এখন করনীয় কি?
পেশেন্ট এর UTI এর কোনো উপসর্গ আছে কিনা, এইটা জিজ্ঞাস করতে হবে, যেমন, Urinary frequency আছে কিনা, Burning sensation আছে কিনা, পেইন আছে কিনা,
যদি কোনো উপসর্গ না থাকে, তাহলে এই কন্ডিশন কে Asymptomatic bacteriuria বলে, অর্থাৎ Urine এর মধ্যে Micro organism আছে, তবে পেশেন্ট Asymptomatic,
যদি কোন পেশেন্ট এর এমন Asymptomatic Bacteriuria থাকে, তাহলে পেশেন যদি Non pregnant হয়, অথবা immuno compromised না হয়, তাহলে কোনো চিকিৎসা লাগবেনা, কোনো এন্টিবায়োটিক লাগবেনা, জাস্ট পেশেন্ট কে বেশি করে পানি খেতে বলবেন, আর কিছুই লাগবেনা,
What is immuno compromised?
ইমিউনোডেফিসিয়েন্সি, যা ইমিউনোকম্প্রোমাইজেশন নামেও পরিচিত, এমন একটি অবস্থা যেখানে সংক্রামক রোগ এবং ক্যান্সারের সাথে লড়াই করার জন্য ইমিউন সিস্টেমের ক্ষমতা আপস বা সম্পূর্ণ অনুপস্থিত। বেশিরভাগ ক্ষেত্রে রোগীর ইমিউন সিস্টেমকে প্রভাবিত করে এমন বহিরাগত কারণগুলির কারণে অর্জিত হয়। অর্থাৎ রোগ প্রতিরোধ ব্যবস্থা অতি দুর্বল। এত দুর্বল যে বহিরাগত ভাইরাস ব্যাকটেরিয়া আক্রমণ হতে শরীরকে রক্ষা করতে অথবা প্রতিরোধ ব্যবস্থা করতে ব্যর্থ হওয়াকে ইমিউনো কম্প্রোমাইজড বলে।
আর যদি পেশেন্ট প্রেগন্যান্ট হয়, অথবা immuno compromised হয়, সেই ক্ষেত্রে পেশেন্ট কে একটা এন্টিবায়োটিক দিবেন,
Asymptomatic bacteriuria in non pregnant = No need any antibiotics.
Asymptomatic Bacteriuria in pregnancy or immuno compromised= Antibiotics needed.
Rx: Nitrofurantoin is the drug of choice in pregnancy in first and Second term.
Term এ Nitrofurantoin দেওয়া যাবেনা, তখন Cefuroxime দিবেন।
Note for a parameter:-
If there are many pus cells in urine R/E, it is called pyuria. If a patient has pyuria, then urine C/S should be done. If urine C/S reveals no growth, it is called sterile pyuria. There are lots of cause behind sterile pyuria.
On the other hand, if urine C/S reveals growth of organism but the patient is totally asymptomatic, this condition is called asymptomatic bacteriuria.

Answer is:-

Biofilm Formation: Bacteria in chronic infections can sometimes form biofilms, which are communities of microorganisms encased in a protective matrix. These biofilms can shield the bacteria from detection in standard urine tests, leading to normal pus cell counts despite the presence of infection.
Low Bacterial Load: In chronic infections, the bacterial load may fluctuate, and it might not always be high enough to cause a significant increase in pus cells in the urine.
Immunosuppression: Individuals with compromised immune systems may not mount a robust inflammatory response to the infection, resulting in fewer pus cells being present in the urine.
Atypical Presentation: Chronic bacterial infections can sometimes present with atypical symptoms or laboratory findings, including normal pus cell counts in the urine.
Sampling Issues: Urine samples collected for testing may not accurately reflect the infection due to improper collection techniques, dilution, or other factors.
In such cases, healthcare professionals may need to consider alternative diagnostic methods, such as urine culture or imaging studies, to confirm the presence of a chronic bacterial infection. Additionally, a comprehensive medical history and physical examination can provide valuable clues for diagnosis and treatment.
