আমাদের নানী বা দাদী দের বয়স হলে হঠাৎ করে ,কোনো কারনে পড়ে গিয়ে; আর আগের মত দাঁড়াতে পারেন না!!!!!এর একটা অন্যতম কারণ হলো 𝙁𝙧𝙖𝙘𝙩𝙪𝙧𝙚 𝙤𝙛 𝙩𝙝𝙚 𝙣𝙚𝙘𝙠 𝙤𝙛 𝙩𝙝𝙚 𝙁𝙚𝙢𝙪𝙧

কেনো কেমনটা হয় বয়স্কদের

অন্যতম কারণ হলো ,দাদি নানী দের 𝐌𝐞𝐧𝐮𝐩𝐚𝐮𝐬𝐞 হয়ে যাওয়ার জন্য ,মানে পিরিয়ড অফ হয়ে যাওয়ার জন্য Body তে 𝐄𝐬𝐭𝐨𝐠𝐞𝐧 𝐇𝐨𝐫𝐦𝐨𝐧 খুবই অল্প পরিমাণে Release হয় আবার অনেক সময় হয় ই না যার জন্যে দেহে Hormonal Imbalance দেখা দেয়!!! So 𝐂𝐚𝐥𝐜𝐢𝐮𝐦 উৎপন্ন ও কমে যায়

যার জন্য Bones এর Density ও কমতে থাকে,,, Ultimately Esily 𝐅𝐫𝐚𝐜𝐭𝐮𝐫𝐞 হয়ে যায় !!!!

আমরা কিভাবে বুজবো যে এইটা আসলে 𝐅𝐞𝐦𝐮𝐫 𝐞𝐫 𝐍𝐞𝐜𝐤 এ fracture হইছে

𝐒𝐢𝐠𝐧 𝐒𝐲𝐦𝐭𝐨𝐦𝐬:-

ꜱᴇᴠᴇʀᴇ ɢʀɪᴏɴ ᴘᴀɪɴ হবে & Front of the Thing also!!

পা 𝐑𝐨𝐭𝐚𝐭𝐞 করতে পারবে না ,Pain হবে।

Injured পা normal পা থেকে কিছুটা খাটো থাকবে

Injured Side দিয়ে ভারী কিছু তুলতে পারবে না(Inavility to Bear weight)

Affected Side এ 𝐒𝐰𝐞𝐥𝐥𝐢𝐧𝐠 হয়ে থাকবে এবং এই পা দিয় কিছু করতে পারবে না

রোগীর কোন Attendant থেকে History নিলে অনেক সময় জানা যায় ,Patient Washroom e যেয়ে নিজের ভার সামলাতে না পেরে পড়ে গেছে;এবং এই অবস্থা হয়েছে। বা কোনো উচু নিচু জায়গায় নামতে উঠতে যেয়ে পড়ে গেছে ,,,সিঁড়ি দিয়ে চলাচলের সময় এমন্টা বেশি হয়

Femoral neck fractures classified into 4 grades of severity using the “Garden” system:–

𝐆𝐚𝐫𝐝𝐞𝐧-1: Partial fracture ( এখানে femoral neck compley broken হয় না -(Incomplete fracture of neck)

𝐆𝐚𝐫𝐝𝐞𝐧-2: এখানে Complete fracture হয় কিন্তু Non Displaced!

𝐆𝐚𝐫𝐝𝐞𝐧-3:Complete fracture হয় এবং Broken ends of the bone have moved out partially..মানে minor Displacement হবে

𝐆𝐚𝐫𝐝𝐞𝐧-4: Complete fracture with separation or Displaced

𝙋𝙖𝙪𝙬𝙚𝙡 𝘾𝙡𝙖𝙨𝙨𝙞𝙛𝙞𝙘𝙖𝙩𝙞𝙤𝙣:-

Type I :- less than 30 degrees

Type II :- 30 to 50 degrees

Type III :- Greater than 50 degrees

Investigation কি দেয়া যেতে পারে

– 𝐗-𝐑𝐚𝐲 of pelvis including Both Hip joint (A/P &Lateral view)

-𝐂𝐓 𝐬𝐜𝐚𝐧 – Fr better classify the fracture pattern or delineate a subtle fracture line.

-𝐌𝐑𝐈-Normally দরকার হয় না Bt May b used to evaluate for femoral neck stress fractures.

• 𝐓𝐫𝐞𝐚𝐭𝐦𝐞𝐧𝐭:-
• Pain remove korar Jonnho NSAIDS DITE HOBE
• Tablet 1.Tory 60 mg 1+0+1 For 5 Days Am.(AS A Etoricoxib As a cyclooxygenase-2 (COX-2) specific inhibitor As a Non-steroidal Anti-inflammatory Drugs (NSAIDs).
• Capsule 2:-Pregaba 25 mg (AS A Pregabalin As a Adjunct anti-epileptic drugs, Primary anti-epileptic drugs.)
• OR
• Tablet 3.Relentus 2mg for 5 days. (As a Tizanidine Hydrochloride As a Centrally acting Skeletal Muscle Relaxants).

𝐘𝐨𝐮𝐧𝐠 𝐏𝐭 হলে ট্রিটমেন্ট with Emergency open reduction internal fixation(Age <60)

𝐄𝐚𝐫𝐥𝐲 𝐏𝐭এর

𝐃𝐢𝐬𝐩𝐥𝐚𝐜𝐞𝐝 𝐟𝐫𝐚𝐜𝐭𝐮𝐫𝐞𝐬 এ Trtmnt depend করে Pt baseline Activity,Level &Age !!

Less Active হলে-Hemiarthroplasty

More active হলে treated with total Hip Arthroplasty

𝐍𝐨𝐧-𝐝𝐢𝐬𝐩𝐥𝐚𝐜𝐞𝐝 হলে

Percutaneous cannulated screws or sliding hip screw

𝐂𝐨𝐦𝐩𝐥𝐢𝐜𝐚𝐭𝐢𝐨𝐧𝐬:-

-Avascular necrosis

-Nonunion

Dislocation increased with total hip arthroplasty surgery

-Osteoarthritis At the neck of the femur & Hip
