Rectal and Colorectal Disease Case no 1:-Anal Fissure.

মাহিন ২৩ বছর বয়স, সমস্যা হচ্ছে, তার পায়খানার রাস্তার আশেপাশে চুলকায়, টয়লেটের পরে টিস্যু পেপার দিয়ে পরিষ্কার করতে গেলে টিস্যু পেপারে রক্ত লেগে যায়, Fresh blood, পায়খানার সাথে রক্ত যায়না,
তবে তার কিছুদিন থেকে কোষ্ঠকাঠিন্য আছে, যখন কোষ্ঠকাঠিন্য, তখন এই সমস্যা দেখা দেয়, কয়েকদিন পর আবার ভালো হয়ে যায়, তাই সে খুব একটা গুরুত্ব দেয়নাই,
এখন গত কিছুদিন তার পায়খানার রাস্তার আসেপাশে চুলকায়, সে ক্রিমির সমস্যা ভেবে ক্রিমির ঔষধ খেলো, কিন্ত উল্যেখযোগ্য কোনো পরিবর্তন নাই, পায়খানার পর চুলকায়, টিস্যু পেপার লাগালে ব্যাথা হয়, সাথে কিছুটা ব্লিডিং হয়-
পায়খানার পরেও মনে হচ্ছে পায়খানার রাস্তার আসেপাশে চুলকায়- এইসব সমস্যা নিয়ে আমার সাথে যোগাযোগ —
তার সমস্যা শুনে মেইন পয়েন্ট যা পেলাম- তাতে ডায়াগনোসিস Anal fissure –
pain + itching after defecation :

চিকিৎসা – নিয়মিত ১২-১৪ গ্লাস পানি পান করুন-

ফাইবার জাতীয় খাবার, ইসবগুলের ভুসি নিয়মিত

Management:–

Topical : Anustat oinment BD dose (১ দিনেই symptoms improve করবে, 4 সপ্তাহ)

(5mg+5mg+10mg+10mg)/gm 15gm tube (AS A Cinchocaine+Hydrocortisone+Neomycin+Esculin) (AS A Compound steroidal preparations, Drugs used in Ano-rectal region)

Indications Of Topical :– Anustat oinment(5mg+5mg+10mg+10mg)/gm 15gm tube (AS A Cinchocaine+Hydrocortisone+Neomycin+Esculin) (AS A Compound steroidal preparations, Drugs used in Ano-rectal region):-

This is indicated for-

Internal and external hemorrhoids
Post-partum hemorrhoidal condition
Anal pruritus, peri-anal eczema, anal fissures and proctitis
Post-hemorrhoidectomy application to relieve pain and discomfort

Side Effects Of Topical :- Anustat oinment(5mg+5mg+10mg+10mg)/gm 15gm tube (AS A Cinchocaine+Hydrocortisone+Neomycin+Esculin) (AS A Compound steroidal preparations, Drugs used in Ano-rectal region):-

Side effects which have been reported for individual constituents may occur and appropriate precautions should be taken when using Anustat.

Pharmacology:-

Cinchocaine is a local anesthetic which relieves severe pain and relaxes sphincteric spasm. Hydrocortisone acts as an anti-inflammatory and anti-pruritic agent and thereby eliminates itching and inflammation. Neomycin is a broad-spectrum antibiotic which eradicates most infections which may already be present or arise in lesions of the anorectal area. Esculin improves skin vasculature and is effective in management of cellulitis.

2. Rectocare ointment (4 সপ্তাহ)

Ointment2:- Rectocare 0.4mg 15gm Tube (As a Nitroglycerin AS a Drugs used in Ano-rectal region).

Indications Of Ointment2:- Rectocare 0.4mg 15gm Tube (As a Nitroglycerin AS a Drugs used in Ano-rectal region):-

The ointment will help to relieve the symptom of pain caused by chronic anal fissures.

Side Effects Of Ointment2:- Rectocare 0.4mg 15gm Tube (As a Nitroglycerin AS a Drugs used in Ano-rectal region):-

Headache and dizziness is a common side effect experienced with Rectocare 0.4% ointment. A possible side effect is reduced control over bowel movements and leaking of bowel motions although this is expected to be a rare event.

Pharmacology:-

The principal pharmacologic action of glyceryl trinitrate is mediated via the release of nitric oxide. When glyceryl trinitrate ointment is applied by the intra-anal route,the internal anal sphincter becomes relaxed.Hypertonicity of the internal but not the external anal sphincter is a predisposing factor in the formation of anal fissures.

পেশেন্ট কে ২ দিন পর কন্ডিশন জানাতে বলি-

সে আজ জানালো, তার এখন itching and pain নাই,

ব্লিডিং ও নাই- তবে এই পেশেন্টের জন্য সবচেয়ে গুরুত্বপূর্ণ যেটা সেটা হচ্ছে এমন সব খাবার পরিহার করা যেগুলিতে কোষ্ঠকাঠিন্য হয়, বিশেষ করে গোস্ত, ডিম ভাজা, তৈলাক্ত খাবার, কাচ্চি, বিরিয়ানী, নান রুটি, এই খাবার গুলি থেকে বিরত থাকা, এই খাবার গুলি থেকে কোষ্ঠকাঠিন্য হয়, সেখান থেকে অ্যানাল ফিশার, হেমোরয়েড /পাইলসের সমস্যা হয়–

এই চিকিৎসার পরেও না কমলে Refer to Colorectal surgeon.

Generel Quaries 1:- রেক্টোকেয়ার ব্যবহারের সময় সতর্কতা অবলম্বন করবে, কারন, মাথাব্যথা হয়। হিপ বাথ বেশ উপকারী ব্যথা কমাতে।
